<p>Background and Objective: This study aimed to investigate and test a structural model of human errors based on safety climate, incorporating the mediating roles of occupational fatigue (acute and chronic) and psychological workload among employees in hazardous industries. Methods: This applied research was a descriptive-correlational study utilizing structural equation modeling (path analysis). The statistical population comprised workers in high-risk industrial environments, from whom a final sample of 398 participants was selected using multi-stage cluster sampling after data screening. Data were collected using the Safety Climate Questionnaire (SCQ), the Occupational Fatigue Exhaustion Recovery (OFER) scale, the NASA Task Load Index (NASA-TLX), and a human error questionnaire. Data analysis was performed using SPSS version 26 and AMOS version 24. Results: Path analysis results indicated that the proposed structural model had a favorable fit (χ2/df = 1.48, RMSEA = 0.035, NFI = 0.986, and CFI = 0.995). Safety climate exerted a direct, negative, and significant effect on acute occupational fatigue (β = -0.221, p &lt; 0.001), chronic occupational fatigue (β= -0.254, p &lt; 0.001), psychological workload (β= -0.368, p &lt; 0.001), and human errors (β= -0.318, p &lt; 0.001). Conversely, acute fatigue (β= 0.201, p = 0.002), chronic fatigue (β= 0.264, p = 0.002), and psychological workload (β= 0.278, p &lt; 0.001) showed direct, positive, and significant effects on human errors. Bootstrap analysis confirmed that occupational fatigue and psychological workload significantly mediated the relationship between safety climate and human errors. Overall, the model accounted for 34.4% of the variance in human errors (R2 = 0.344). Conclusion: The findings demonstrate that improving the safety climate not only directly reduces errors but also plays a vital role in enhancing operational reliability and preventing industrial accidents by mitigating occupational fatigue and psychological workload.</p>
